We are happy to assist you by providing receipts upon request. On a receipt, we will document a diagnosis (if applicable), your identifying information (such as name, date of birth, address, etc.), session information (CPT codes), and payment details. It is your decision whether to share this insurance receipt with your insurance company. You should also be aware that if you ask me to complete an insurance form, you are authorizing me to provide the data that is requested. Any information provided to your insurance company will be part of your medical file.
